Silicone Plugs
Silicone plugs are semi-permanent and can last for years, but are not the best option in my opinion. The top piece of the plug rests on the eyelid and can cause discomfort if it rubs on the eye. They also tend to collect makeup, debris and bacteria over time which can actually increase irritation.
Collagen Plugs
Collagen punctal plugs are inserted into the tear duct and since they do not sit on the eyelid, they won't cause any discomfort. They dissolve within a few weeks to 6 months and can easily be replaced. Inserting them is quick and painless, making them a great option for some patients. Permanent punctal cautery is also an option but I prefer plugs so they can be reversed if tearing develops.
A New Treatment…
Lacrifill Canalicular Gel is a new intervention that can help treat dry eye. It is a novel approach, using hyaluronic acid gel to fill the tear drainage system, increasing the amount of tears on the surface of the eye. This just became available and we are excited to start using it!
When are Punctal Plugs a Good Option…
Plugs are not a good option for all patients. They increase the tear volume on the surface of the eye, which sounds good, but diving deeper this can also increase inflammation. I only place them in patients that don’t have signs of inflammation and have negative Inflammadry testing, otherwise they can worsen burning and irritation..
This video is a great example of why I never place silicone plugs anymore. This silicone plug was placed many years ago before the patient saw me, is not seated well and is full of gunk! Unfortunately, it could not be removed in the office and isn’t bothering the patient so was left in place. You can see how easy it is to place a collagen plug instead which will gradually dissolve over time and never need to be removed.
